Step 2: Create a Dragonfly account. Rising 8th Graders that are in DragonFly already –  you will need to fill out all of the DragonFly paperwork again. Make sure you select the 2024-25 year as well as Leesville MS as your school even if you attend Hilburn you must select Leesville MS.

Choose ALL sports you are interested in trying out for.

1. 1. Once you have the athlete’s page, scroll down to the bottom and click “Get Started”. This will guide you through the whole process. There are 2 forms that need to be printed out and taken to the doctor (Physical Examination Form and Medical Eligibility Form). You can print the forms from this site. As soon as the Doctor has fully completed each form, you will upload each form from the doctor into their proper place on Dragonfly.
   2. Be sure all boxes are green or it says pending approval. We will approve them as soon as possible after all parts to Dragonfly have been completed.  If you have not been approved, something is incomplete or missing.
   3. Check Dragonfly periodically until we have approved all your paperwork and you have all green boxes.

1. You must be in 7th or 8th Grade.
2. Must not be 15 yrs old before Aug. 31, 2024.
3. Earn Passing grades in 3/4 Core Classes from the previous semester.
4. Must be promoted to next grade.
5. May not have more than 14 absences in the previous semester.  There is no difference in excused or unexcused.  (Must attend 85% of classes in previous semester.)
6. Students must be in compliance with 6 semester rule, meet age requirement (listed above) and be promoted to the 2024-2025 school year.
7. Must have completed all parts on Dragonflymax including a PPE Physical Examination Form and PPE Medical Eligibility Form that have Doctor signatures; their office stamp; and date of exams that are each properly downloaded into Dragonfly.
